**Job Summary**

Position is responsible for oversight of all inpatient rehabilitation clinical admissions and referrals. In addition, assists in coordinating inpatient rehabilitation admissions across the Memorial Hermann Rehab Network.

**Location: TIRR**

**Address: 1333 Moursund Avenue, Houston, TX 77030**

**Shift: Onsite Monday to Friday 8AM -5PM (potential weekends)**

**Minimum Qualifications**

**Education**: Associates Degree of Nursing required; Bachelors preferred

**Licenses/Certifications**: Current Texas RN license required. CRRN and CCMC preferred

**Experience / Knowledge / Skills**:

- Three (3) years of successful work experience in an IRF and/or Rehab environment
- Excellent critical thinking skills and ability to manage multiple projects independently

**Principal Accountabilities**
- Supports a team of rehabilitation admission nurse liaisons who assess appropriateness and provide medical clearance for patients in in the referring facility (SNF/LTAC/acute/home) for acute inpatient rehab. This position does not supervise bedside staff or any staff who provide clinical care.
- Collaborates with the full Admission team, staff and leaders across TIRR and the MH Rehabilitation Network.
- Collaborates with rehab nursing leaders to determine the network's ability to provide care for identified patients.
- Performs rehabilitation pre-admission screens and medical updates as needed.
- The nursing staff in this area do not provide direct hands-on patient care.
- Establishes goals/objectives and trains, monitors, and evaluates staff as appropriate.
- Ensures safe care to patients, staff, and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity, and quality of service.
- Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff.
