**Position Summary**:
Maintain company-wide wellness program. Creates awareness, and motivation and provides tools to employees that help them adapt and maintain a well-rounded, healthy lifestyle. Provide education, coaching curriculum, and coaching to help participants identify their motivations, and wellness visions, and build care plans that will help them achieve their long-term goals. This role will be responsible for current health issues, preventative care, and health maintenance follow-up to improve health.

**Essential Duties and Responsibilities**
- Assessment of medical reports for patients and calculation of scores for employees; evaluate the need for continued follow-up with employees based on scores from physician reports.
- Educate and counsel patients emphasizing personal motivation to improve behaviors, education and awareness to improve the lifestyle behaviors as well as set goals, answer questions, and provide guidance and ongoing support
- Maintain, coordinate, and update wellness program information and procedures to meet client specifics.
- Utilize the established template and documentation standards
- Prioritize and coordinate multiple responsibilities
- Work closely with the clients to update them on the benefits of the program and identify trends
- Maintain confidentiality and compliance with Federal, State, and local regulations pertaining to HIPAA
- Other duties as assigned by the supervisor
- Some travel may occasionally be required to meet with clients

**Education**:

- Certified Health Coach from an accredited organization (NCHEC or NBHWC) Exercise Physiologist, or
- RNs or LPNs with current unrestricted licenses, prefer Wellness and Disease Management experiences
- College degree preferred, with a minimum 2 years of health coaching experience, or related experience.

**Experience**:

- Two or more years of recent Wellness Coaching experience.
- Experience working with chronic condition management, such as diabetes, hypertension, weight management, etc.
- Bilingual preferred

**Additional Functions and Responsibilities**:

- Organized, detail-oriented team player with an ability to multitask and a fast learner in unfamiliar areas.
- Demonstrates ability to meet administrative requirements, including productivity, time management, and Quality Assurance standards.
- Very strong social and relationship-building skills
- Comfortable making cold calls and talking to new people all day.
- Excellent customer service skills to resolve participant concerns calmly and professionally with coaching and/or technology.
- Strong verbal and written communication skills
- Maintain confidentiality
- Knowledge of laws and regulations pertaining to HIPPA and PHI
- Must be open to feedback and have a desire to continuously learn and grow.
