Admission Coordinator

**Duties**:
- Respond to inquiries regarding potential nursing home admissions. Inquiries may be in writing, by telephone, or in person. Provide information verbally and/or in written about the admissions process and tours of the facility.
- Receive inquiries/referrals on behalf of potential residents and forwards to the Admissions Nurse upon receipt.
- Verify medical insurance of potential residents (Medicare, Medicaid, other).
- Conducts financial Admission Meeting with family, gathers appropriate information and signatures using the admission checklist as a guide, and prepares the financial folder of the resident. Once information is gathered, ensures that all information is distributed to the appropriate departments or agencies and that all information is filed in the appropriate location (ie. medical record, financial file, etc.).
- Complete Social Security Representative payee forms when appropriate.
- Inform resident prior to or upon admission of rules and regulations governing resident conduct, responsibilities and rights.
- Complete appropriate Medicare denial letter, verification, or SNF letter as necessary upon admission.
- Enters Admission into LTC System.
- Notifies the Nursing Department of pending admissions, ensures that the Resident’s name is on the door of the assigned room.
- Sends an admission notice to all departments to notify them of the pending admission.
- Delivers the admission orders to the appropriate unit prior to the arrival of the resident.
- Other duties as assigned.
