This position works within our Support Coordination Division. Supervises a team of Support Coordinators and provides clinical direction to them in areas including but not limited to: clinical assessment of client needs (including treatment modalities, intervention techniques, behavior management, independent living skills, supportive counseling, family education, crisis intervention, housing, communication, recreation, financial, medical, transportation, vocational, legal, education, etc.); interdisciplinary team leadership; formulating, writing and developing person centered plans; continuous individual progress review and evaluation, use of assessment tools; referral methods; case consultation, documentation, and special projects assists support coordinators in determining clinical interventions and in problem solving in order to enhance their knowledge, and supervises support coordinators administrative duties. Provides training and education regarding inter and intra-agency services and clinical interventions. Oversees and reviews submissions of services that are required to maintain under the DD waivers (service modifications, renewals, etc.) including submissions in WaMS (Waiver Management System). Participates in record and utilization reviews, assume caseload duties in the absence of supervisees, provides crisis intervention assistance, participates in Emergency 24-Hour on-call rotation for service provision, oversees the use and maintenance of the electronic health record to record all aspects of support coordination performance as well as to document the delivery of services.
