We are continuing to grow and expand our services offered for Lake County residents with a focus on offering a supportive team environment for employees and volunteers daily.

Specialized training, coaching, and support are provided as needed for clinical team members.

The Registered Nurse Case Manager plans, organizes, and coordinates care under the Medical Director's orders with other Interdisciplinary team members. Care and family support are provided in the patient’s place of residence whether it is a private home, a residential care facility, or a skilled nursing facility.

Work hours are Monday-Friday pm from 8:00 am-4:30 with an occasional evening on-call coverage requested and a 1-in-6 Saturday daytime schedule. A 32-hour work week is also allowed.

Current California RN license and a minimum of three years of experience preferred. Valid driver’s license, reliable transportation, drug screening, and background check will be required.
