We host 30 and 60 day check-ins and then we’ll hold quarterly development meetings that will drive personal and professional growth through objective goals, with plenty of room for advancement throughout the team. Every member of our care coordination leadership team, including Leads, Managers, and our Director, have started as Patient Care Coordinators and have been promoted from within. You will also participate in annual performance reviews that will provide opportunities for two way feedback, as well as compensation adjustments.

In this role, you will be responsible for:

- Scheduling patient appointments, managing cancellations and encouraging rescheduling whenever possible;
- Handling new patient intake calls; accurately inputting demographic and insurance information into our scheduling software; assessing patient concerns to ensure they are booked with the appropriate PT/OT;
- Staying informed and being transparent with patients about their financial responsibility; collecting any outstanding balances;
- Managing patients’ prior authorization and referrals, requesting additional visits when needed and notifying patients when there may have a lapse in coverage;
- Providing an excellent client experience by taking the necessary time to understand the unique needs of our patients while remaining patient and empathetic when coordinating care.

Hours for this position are **Monday, Wednesday: 10:40am-7:20pm; and Tuesday, Thursday, Friday: 6:20am-3pm**.

Our hiring process consists of three interviews (one phone, one virtual, one in-clinic), and two professional reference checks.

Pay: $18.00 - $20.00 per hour
