The Outpatient Counselor will be responsible for providing direct clinical services to individuals and family members who have been affected by chemical dependency, psychiatric problems and/or co-dependency issues and will function as part of multi-disciplinary treatment team. Maintain clinical record in compliance with all licensing, regulatory and accrediting agency requirements.
- Functions as a Case Manager for existing patient caseload.
- Responsible for maintaining clinical chart including, but not limited to Intake and Assessment, Diagnostic Summary, Treatment Plans, Progress Notes, Consult notes, Discharge Summaries, Continuing Care Plans and other documents as is required.
- Develops patient focused, meaningful treatment plans.
- Provides diagnostic intake assessment & evaluation utilizing ASM and PCPC criteria.
- Ensures competent management of co-occurring treatment needs.
- Coordinates services, including precertification, concurrent reviews, and making necessary referrals within confidentiality guidelines and practices.
- Conducts educational seminars to patient population in a professional and competent manner.
- Provides individual, group, couple and family counseling sessions to patient population and their families in a professional and competent manner.
- Facilitates psychoeducational groups.
- Attends all staff meetings, trainings, and committees as requested.
- Assists in marketing functions when appropriate.
- Responsible to meet all Federal, State and Local confidentiality requirements at all times.
- Performs other related duties as may be required or as assigned.
- A Master’s degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services) or other related field which includes a practicum in a health or human service agency preferably in a drug and alcohol setting.
- Must hold a professional license through the PA Department of State or be credentialed through PCB at the time of hire or be actively working towards licensure or certification. A PCB credential must be obtained within two years of hire or licensure within 4 years of hire.
- Knowledge of Substance Use Disorder (SUD) treatment and/or ASAM criteria.
