Blize Healthcare is looking for an **Intake Coordinator **to join our team.

**Job Functions/Responsibilities**:

- Handle daily patient referral & intake operations.
- Ensure compliance with all State, Federal, and Joint Commission referral/intake regulatory requirements.
- Establish and maintain positive working relationships with current and potential referral sources.
- Ensure the seamless transition of patients to home care.
- Perform office duties such as answering telephones (multi-line phone system and making appointments or calls to the appropriate party) completing insurance forms and answering any patient inquiries.
- Consult with a physician regarding incomplete patient tests/consults or other incomplete orders before the patient visit.
- Create, maintain, copy, and file patient files, records, and other information as needed.
- Request for authorizations, follow up on denials and correspond with Adjusters to answer any questions.
- Verify insurance coverage and assist the billing department with processing claims.

**Knowledge Skills & Abilities**:

- 1 year of medical experience preferred.
- Fluency in a language other than English, a plus.
- Considerable knowledge of principles and practices of medicine.
- Proficient in Microsoft Office (Word and Excel).
- Excellent interpersonal skills.
