**Job Summary and Qualifications**:
As a Valve Program Coordinator you will plan, coordinate, and implement the care of the valve patient in the outpatient setting. In addition, you will act as a liaison with the physicians, patient/families and ancillary departments.
- Plan, coordinator, and implement care of the valve patients in the outpatient clinic
- Review clinical progress of patients and coordinates communications to referring, valve and primary care physicians
- Plan, organize and provide direct care to the valve patient
- Facilitate the development of a patient specific teaching plan
- Review patient’s current health information, relay information and make referrals as necessary to the physician
- Serve as a resource for the hospital staff regarding the care of the valve patient
- Other duties as assigned
**Requirements**:
Experience
- Minimum of 2 years of experience as a Registered Nurse (RN) working with cardiac patients
Education
- Associate's degree in Nursing (ADN) from an accredited nursing program
- Bachelor of Science in Nursing (BSN) - preferred
License and Certifications
- Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License Texas license must be obtained within 90 days of hire
- Basic Life Support (BLS) required as per St. David’s Healthcare policy
- Advanced Cardiovascular Life Support (ACLS) certification - preferred
