You have a current **unrestricted RN license in the state of Illinois?**:

- You have at least **1 year of paid RN Experience?**:

- You have a valid **TB Clearance, CPR Certification, & Work Physical?**:

- You understand regulatory compliance with state and federal regulations?
- You have a valid D.L. & Vehicle with Insurance?

**Available Shifts**
- ** [7pm - 7:30am]**:

- Patients in Hospice Facility
- 8 hour shift training provided for first week

**Responsibilities**
- Port/PICC line, TPN, IV Pain meds, and Fluids/TPN
- Provide thoughtful care to client & patients based on personalized Plan of Care
- Monitor and track changes in the client/patient’s conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
- Proactively updates client/patients’ family on plan of care, health changes, and concerns
