**JOB TITLE**: Facility Manager

**REPORTS TO**: QIDP/RN/Administrator

**OVERVIEW OF POSITION**:
The Facility Manager will work with individuals with developmental disabilities in a community based, residential health care facility (ICFID-H/ICFID-N), performing a variety of functions as assigned by the above. The FM will serve part of the management team at the facility and will perform all necessary duties pursuant to the job responsibilities. Finger print clearance and successful completion of physical examination and drug/alcohol tests are required. The FM must be at least 18 years of age and have graduated from High School or completed requirements of the G.E.D. The FM will possess 18 months experience working with individuals with developmental disabilities, or 6 months experience working with individuals with developmental disabilities plus 30 college units in a related field.

**RESPONSIBILITIES/DUTIES**:
**General Duties**:
1. Personal/Staffing:

- Assist with Staff scheduling
- Monitoring staff tardiness and absences
- Conduct orientation with new employees
- Continue on-going training with new staff and current staff
- Conduct face to face in-services and in Therap
- Notify upper management of any house concerns.
- Assist with time sheets
- Staff discipline and monitoring
- Ensure staff adherence to policies and procedures
- Monitor staff/facility compliance with all regulatory agencies
- Complete all duties in Therap (T-Logs, GER’s, appts, vitals, scomms,), etc
- Monitor staff requirements for 90 day and annual evaluations.
- Audit client information in Therap weekly/monthly
- Assist with facility coverage when shortage of staffing
- Attend ISP and HRC meetings
- Administer client medication and treatments
- Conduct facility checks weekly
- Check for expired medication and food
- Transportation for appointments and activities
- Notify upper management of emergency situations
- Attend surveys exits if within schedule time frame and approved
- Ensure all clients having adequate supplies in their back for day program
- Notify upper management of unsafe situations
- Monitor fire and disaster drills being conducted in the home
- Attend house meetings
- Report any deviations from staff scheduling by staff
- Submit any mileage report to upper management
- Work with upper management for successful management of budgeting
- Directly responsible for consumers and staff at assigned facility
- Ensure all staff have orientation to work in the home and follow up to ensure all staff have proper certification to work in the facility
- Be a role model and lead by example.
- Perform any other duties as assigned by upper management

2. Appointments
- Schedule and manage all clients’ appointments. Make sure all appointments are in Therap in a timely manner. Follow up the day before to ensure that the appointment is still valid.
- Notify QIDP and the RN or results of a client appointment
- Arrange transportation for clients appointments
- Take clients to scheduled appointments
- Follow up on all necessary information (call off’s,/staff charting/recommendations), etc

3. Active Treatment
- Submit request for any recreational activities
- Follow through on scheduled activities
- Scheduled activities weekly/monthly
- Maintain a thorough knowledge of all consumers information
- Supervise staff to ensure proper and consistent implementation of active treatment program for each consumer
- Participate and role model active treatment in the facility
- In-service staff for proper implementation of all active treatment for all clients
- Ensure clients are dressed and groomed appropriately

4. Supplies/Forms
- Check and complete supply forms in a timely manner
- Check the facility for the following (nursing items/deodorant/soap, toothbrush, toothpaste/diapers/cleaning supplies/linens/towels/pillows/plates/silverware/pots and pans/office supplies/program and recreational supplies, etc.

5. Consumer Purchases
- Personal and clothing
- Reconcile and turn in receipts and documentation asap.
- Keep P&I book up to data, with receipts and calculations.

6. Medical Records
- Ensure all tasks such as job duties and responsibilities being done by all shifts
- Ensure all models and being completed per all staff (T-logs, vitals, GER’s, ISP, intake and elimination, time tracking), etc.
- Do med checks.
- Review client medication daily/weekly/monthly
- Check med cart for any expired medications.
- Complete all duties before end of shift

7. Physician Services
- Participate in Doctors Day.
- Participate in Medication Management Day
- Notify QIDP and RN for any client COC’s
- Follow through with all new orders implemented by the Doctor

8. Incident Reports and Follow Up.
- Timely and proper notification and documentation
- Follow up as needed

9. Physical Plant
- Maintain repair list and submit to the QIDP
- Follow Up with any needed repairs
- Responsible for upkeep and cleanliness of the facility
- Complete fire drills, disaster dr
