**ABOUT THE ROLE**:
At Hopscotch Primary Care, we take a team approach to serve patient needs and provide the best care possible. Our goal is to provide the care each of us would want for ourselves or for our family members, in the right setting, and at the right time. The Hopscotch Care Team is responsible for delivering high-touch, high-quality care to patients, including coordination of care across different physicians and the broader healthcare continuum.

As **Manager**, Population Health**, you will play a critical role in designing and leading implementation of our population health operating model. The role will support core population health domains including but not limited to workflow optimizations, assessing vendor support and other tools, and driving excellence in clinical documentation. This role will report to the Population Health Director.

**WHAT YOU'LL DO**:
Core Responsibilities:

- Assess, refine and develop workflow optimizations to enable best-in-class clinical outcomes for the Hopscotch model.
- Collaborates closely with a cross functional team to build out the Hopscotch population health capabilities that contribute to a best-in-class Hopscotch care model for rural populations
- Develop solutions that scale across multiple locations
- Key leader in data analytics to build population health dashboards that monitor performance and spotlight improvement opportunities
- Own the execution and ongoing refinement of a best in class clinical documentation program.
- Develop and manage the medical scribe program to help aid in clinic workflow efficiencies including accurate documentation
- Develop an ongoing HCC coding education program
- Develop chart auditing process for accurate documentation
- Ongoing evaluation and assessment of tools that help aid in accurate documentation

**ABOUT YOU**:
We are looking for a motivated, mission driven, and experienced leader with:

- 3-5 years of relevant experience in healthcare, population health, working with providers, or other similar experience with strong preference for value-based care models
- Bachelor's Degree required
- Graduate Degree a plus (MBA, MPH)
- Ability to analyze data to build out a business case or ROI analysis
- Experience working with multiple software & data platforms, electronic health records, spreadsheet development and navigation, data processing, organizational skills and ability to manage multiple priorities appropriately
- US work authorization
