**Job Summary**:
Primary function is to process all aspects of scheduling nursing visits via Home Care Home Base software system.
**Functions**:
- Schedule and appropriately document patient schedules in a timely manner.
- Participate in coordinating care with management and patient interdisciplinary team.
- Provide effective communication to patients, staff members, other health care professionals and referral sources.
- Respond to agency and patient needs in a professional and creative manner.
- Demonstrate commitment, professional growth and competency.
- Promote Agency philosophy and administrative policies to ensure quality of care.
**Skills & Experience**:
- **Minimum 1 year of home health experience.**:
- Excellent computer skills required.
- Excellent interpersonal and organizational skills.
- Basic knowledge of scanner/fax/copier equipment.
- Phone etiquette skills required.
- Ability to take direction with a multitude of tasks.
- Typing ability 40-50 WPM.
- Knowledge of Medicare regulatory guidelines.
**Reliable source of transportation and valid/current auto liability insurance is required.**
