Description:
The Spiritual Care Counselor helps achieve our mission by providing spiritual care for Hospice patients and families. The Spiritual Care Counselor works effectively with members of the Hospice team in establishing, implementing, and evaluating the care plan for each family regarding spiritual and bereavement needs. Some essential job functions include:

- Provides spiritual counseling directly to patients and families according to assessment of needs in a manner consistent with patient and family beliefs and desires.
- Communicates with local clergy/pastoral counselors to support patients and family's spiritual needs.
- Completes spiritual assessments and visit information and outcomes according to procedure and professional standards.
- Participate in, facilitate or develop in-service education for Program and Agency staff as required by the manager.
- Participate in the development and ongoing evaluation of the spiritual components of the care plan.

**Other Job Functions**
- Participate in interdisciplinary group meetings and other case conferences as requested.
- Actively participate in team/staff meetings and provide constructive input to the meeting leader.
- Participate in planning for the agency memorial services, patient's memorial services, and facility memorial services as requested and approved by manager.
- Assist in supervision and communication with Spiritual Care Volunteers and students when involved with a patient on your caseload.

**Requirements**:

- Master's Degree in Divinity or related degree and at least 2 units of CPE (Clinical Pastoral Education)
