POSITION: Spiritual & Bereavement Care Coordinator REPORTS TO: Director of Nursing

STATUS: Exempt Non-Exempt

: The Spiritual & Bereavement Care Coordinator, is responsible for developing a comprehensive bereavement program ensuring that patients and families receive necessary bereavement support. Additionally, the
Spiritual & Bereavement Care Coordinator provides Spiritual Assessments, Spiritual Care, and Clergy Services, for patients and families. The Spiritual & Bereavement Care Coordinator assists company staff in managing their grief.

The responsibilities of a Spiritual & Bereavement Care Coordinator revolve around caring for the spiritual needs of each hospice patient and their families. Your duties may include arranging counseling for patients and grieving families, offering spiritual support to caregivers, and helping with funeral planning.

PRINCIPAL ACCOUNTABILITIES:
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Spiritual Care

1. Provide direct spiritual support and/or counsel to patients/families in keeping with patients’/families beliefs;
2. Provide short-term pastoral counseling to patients, families and staff, using age appropriate communication;
3. Serve as a member of the Interdisciplinary Team involved in client care planning, with particular focus on spiritual components of care;
4. Facilitate clients’ and families’ access to spiritual care as requested and desired; Inform clients and their families about available community clergy;
5. Demonstrate competency in the cultural and age specific needs of the clients;
6. Provide or facilitate rituals at times of transition, including death.

Bereavement

1. Ensures the company Bereavement Policy and Procedure is followed and company is complaint with state and federal regulations associated with Bereavement services.

2. Upon admission, identifies bereavement risk, goals and interventions, performing an initial assessment of patient and the patient’s family and/or caregivers.

3. Provides patient and family and/or caregiver with information about company services and support including the availability of clergy.

4. Upon patient’s death, for the family member(s) and/or caregivers(s), performs a post death bereavement assessment and develops a bereavement Plan of Care based on identified needs.

5. Ensures that the bereavement Plan of Care is followed and provides ongoing bereavement assessment.

6. Assumes and maintains accountability for bereavement program compliance with federal and state regulations.

7. Provides bereavement training for volunteers.

8. Facilitates or participates in grief support groups.

9. Conduct memorial services/liturgical assignments for clients who have died providing continuity of spiritual support when requested.

10. Conduct or make arrangements for funeral or memorial service when indicated. Plans and conducts memorial service for bereaved families as requested and at least annually.

11. Has a duty to report to the assigned RN, any changes in patient condition and anytime the patient is in pain.

12. Has a duty to report any potential misconduct or violation of Hospice Standards of Conduct or federal or state laws and regulations.

Administration

1. Develop and supervise the provision of spiritual care to clients and families and assist in design, implementation, and evaluation of the spiritual care program, ensuring proper documentation.

2. Develop and maintain a resource group of clergies to whom specific aspects of spiritual care may be delegated.

3. Process spiritual assessments and referrals for assignment to professional, staff, volunteers and spiritual care volunteers as requested according to need, level of expertise required, and staff available.

4. Arrange for on-call availability of Spiritual Care services.

5. Facilitate and advocate for the cultural needs of patients, families and staff.

Education

1. Work with staff, volunteers, clergy, lay ministers and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.

2. Provide education, support and consultation to clergy in the community regarding end of life issues;
3. Participate and/or lead discussions on ethics related to end of life care;
4. Integrate spiritual care competencies into policies for staff and volunteers in coordination with the Volunteer
Coordinator.

General Responsibilities

1. May be required to perform the responsibilities of the Bereavement Coordinator. See the job description for the Bereavement Coordinator.

2. In coordination with the Director of Volunteer Services recruit train and supervise spiritual care and bereavement volunteers.

3. Conduct memorial services/liturgical assignments for clients who have died providing continuity of spiritual support when requested and when schedule permits.
