**Position**:Case Manager

**Description**:

- Oversee assessment of clients
- Assist in development of client objectives
- Ensure client health and safety issues are addressed at job site
- Set up and monitor client files
- Participate in client service planning meetings
- Write reports on client progress
- Meet with clients to discuss progress
- Communicate effectively with support teams
- Coordinate with consultant services, if any
- May be required to provide direct service to consumers
- May be required to assume Program Director duties in some circumstances
- Other duties as assigned by Program Director or Regional Director
- Mandated Abuse Reporter

**~Bi-lingual (English/Spanish) preferred ~**
