The Case Manager will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary resources and assistance to improve their well-being. **Responsibilities**: - Conduct assessments to identify clients' needs and develop individualized service plans - Provide mentoring and counseling to clients facing various challenges - Coordinate services such as childcare, workforce development, addiction counseling, and social work support - Implement crisis intervention strategies when necessary - Supervise and monitor client progress towards set goals - Collaborate with community resources and agencies to advocate for clients' needs - Assist with housing programs like Section 8 and child welfare services **Skills**: - Proficiency in crisis management and intervention techniques - Strong mentoring and supervisory skills - Experience in workforce development and addiction counseling - Knowledge of child welfare services and childcare resources - Ability to handle complex cases with empathy and professionalism If you are passionate about making a difference in the lives of others and possess the skills required for this role, we encourage you to apply.
