Functions as an advocate and primary service coordinator for assigned caseload of clients, serves as leader of the interdisciplinary team; facilitates Personal Futures Plan meetings; hires, supervises, trains, leads and assist Case Mangers; coordinates service plan implementation; ensures client needs are identified, addressed and met as directed by the Individual Futures Plan and recommended by the interdisciplinary team, all necessary follow‑up is carried out, all Individual Futures Plan documentation is completed and in accordance with the standards set by the Agency and all applicable State and Federal governing, funding and accreditation agencies.

Selects and supervises (orients, trains, schedules, evaluates, commends, disciplines, and directs activities) assigned direct subordinate staff and ensures same for all staff supervised by direct report(s) utilizing measurable performance evaluation criteria for staff growth and ensuring that performance standards necessary to meet the needs of persons served are met, in a manner consistent with the mission and objectives of the Agency and the services it provides.

**QUALIFICATIONS**

Bachelor degree in Social work, Psychology or related field as required by state regulations. Minimum of two years successful work experience with person with developmental disabilities, including one year supervisory experience. Valid Illinois Driver's License. Certificate of Proof of Insurance and maintenance of such at minimum amount of coverage required by law. Possess or acquire within first ninety days of employment, self administration of medication training and defensive driving. Possess or acquire within first year of employment, first aide, C.P.R., updating as required by licensing agency.

Pay: From $47,840.00 per year
