Lavaca Family Health Clinic is searching for a dedicated **LVN** for the clinic chart nurse position. This role requires someone who is highly self-motivated and able to multi-task on a daily basis. The chart nurse role entails (but is not limited to):

- calling and educating patients on their lab and radiology results,
- educating on orders given by the provider,
- effectively working with home health agencies to ensure a flow of communication between the home health nurse and the provider, and
- willingness to speak to patients and patient family members who walk up unexpectedly to the clinic.
- Typical Shift Hours: 8 am-5 pm Mon-Fri

Basic computer and typing skills are required for the electronic health record (EHR) system utilized in the clinic. Previous experience with an EHR is a plus. The chart nurse will also be expected to perform nursing skills such as:

- check-in,
- vital signs,
- injections,
- ear washes, and
- assisting with simple office procedures.

The chart nurse position is the link between a patient and their provider, which creates lasting relationships that are cherished by patients and their families alike.
