**What is Two Hearts Homes For Seniors?**
- Two Hearts Homes for Seniors LLC_ is a unique area of senior care designed to assist residents with activities of daily living while providing privacy and comfort as if they were in their own home.

The home for seniors is a 16-bedroom ranch-style house. Almost everyone who comes to visit says something along the lines of “wow, I thought I was walking into the wrong place This is actually a HOME” Each resident has their own room with a private bathroom and shower in each. The intimacy of the home is for those who require help with daily activities but still want to **feel like they are at home rather than in a large facility**. The open concept of the main area is based on the feeling of family where residents dine together for meals and visit with each other on the comfort of couches just like at home. With 24-hour caregivers present and homemade meals, residents at Two Hearts feel safe and well taken care of. Those living in the home feel supported not only by the caregivers but each other (just like family).

Position Summary: Responsible for the day-to-day operations of Two Hearts homes including staff oversight, building maintenance, resident services, and marketing. Assures compliance with all rules and regulations as well as policies and procedures.

Work Schedule: Varies (days, nights, weekends, holidays)

Reports to: Business Owner / Chief Operating Officer COO Education, Experience, & Qualifications: Prior experience in administration and/or management. Experience in management of assisted living preferred. Experience working with the elderly and providing care for those who require assistance with ADLs. CPR/BLS certified.

CNA preferred. Must speak English and be able to communicate effectively with physicians, staff, outsourced therapies, residents, and family members. Must be able to organize tasks effectively and function autonomously as a team leader. Ability to lift heavy weight and spend long periods of time standing. Must be dependable, well-groomed, follow dress code requirements, and able to learn and accomplish new tasks with mínimal supervision.

Responsibilities (include but are not limited to):
1. Management of Home(s)

a. Ability and willingness to travel between multiple Two Hearts home locations

b. Available on-call to provide consultation to staff as needed and arrange for

appropriate coverage when unavailable

c. Create staff schedule and make changes as needed

d. Maintain as-needed staffing backup for emergencies/ call-ins

e. Schedule in-service continuing education for staff bi-monthly (work with

supervisor/COO)

f. Maintain operation procedures including monthly fire drill. If repairs to the home

are needed, contact appropriate facilities

g. Direct mail to the appropriate persons

h. Maintain resident files and compliance

i. Review patient charts and documentation daily/every other day

j. Manage resident, family, and staff grievances with support of supervisor/COO

k. Coordinate activities calendar and reserve therapies at least two weeks before

activity is to take place

l. Order any supplies that are needed for the home

m. Ensure appropriate utilization of community resources, including the provision of

transportation as needed/as appropriate

n. Oversee the accurate completion of all accounting-related tasks including payroll,

accounts payable, and accounts receivable

2. Resident Care

a. Ensure resident assessments and admissions are performed in a timely manner;
b. coordinating all resident move-ins and paperwork

c. Ensure resident discharges are completed in a timely manner

d. Provide resident care as needed

e. Manage resident assessments and charts periodically to ensure resident safety,

health, and well-being

f. Report to resident family members any change in health condition

g. Monitor for changes in residents and assess for signs of abuse or neglect and

report accordingly

h. Provide emotional support to residents and protect the rights and privacy of

residents

i. Develop resident service plans and maintain compliance for resident care

ensuring plans of care are followed

j. Monitor resident engagement with activities

k. Report all unusual incidents to family and supervisor/COO

1. Staff

a. Support direct care staff

b. Train new employees for the home and pair new caregivers for job shadowing

appropriately

c. Ensure staff is documenting in resident charts correctly

d. Staff evaluations bi-annually/annually

e. Maintain employee files and compliance

f. Perform human resource duties: interviewing new caregivers, setting up training,

reprimand staff, and terminations

g. Employee recognition

h. Assess employee compliance with monthly competencies (HCP)

1. Food Service

a. Modify menus to meet dietary requirements of residents

a. follow monthly menus in binders and change per season (Grove

Menus)

b. Prepare and order food and supplies needed by the house

c. Ensure menus are foll
