Job Overview:
We are seeking an experienced and dedicated Advanced Practice Registered Nurse to join our healthcare team. As an Advanced Practice Registered Nurse, you will play a crucial role in providing high-quality patient care and ensuring the well-being of our patients. This position requires strong clinical skills, excellent communication abilities, and a passion for delivering compassionate healthcare.

**Responsibilities**:

- Conduct comprehensive patient assessments and develop individualized care plans
- Perform utilization review to ensure appropriate and efficient use of healthcare resources
- Administer medications and monitor patients for adverse reactions
- Provide education and support to patients and their families on disease management, Chronic Care Management, HEDIS measures, treatment options, and preventive care
- Coordinate with other healthcare professionals to ensure continuity of care
- Assist with durable medical equipment (DME) ordering and management
- Assist with Wound Care, Stitch removal, IVs
- Perform venipuncture and collect laboratory specimens as required

**Requirements**:

- Valid Advanced Practice Registered Nurse license in Florida
- Master's degree in Nursing or related field
- Certification as an Advanced Practice Registered Nurse (APRN)
- Minimum 1 years of experience in a similar role
- Proficient in utilizing electronic medical record systems, eClinicalWorks a plus
- Strong knowledge of medical terminology, procedures, MRA documentation, HEDIS measures, and best practices
- Excellent clinical assessment and diagnostic skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Exceptional communication and interpersonal skills, and a Team player.
