We are looking for a Fulltime, Patient Care Coordinator for the Mission Hills branch. LVN preferred but not required.

$25-$30 per hour without an LVN license DOE

$30-$35 per hour with an LVN license DOE

**Who you are**:

- Prepares nursing call schedule.
- Receive updated orders from physicians
- Takes and prepares intake referrals.
- Follows up on patient grievances.
- Participates in team and goals of the Agency.
- Participates in weekly case conferences when available.
- Requests authorization of utilization from third party payors.
- When requested, gives a report to case managers of third-party payors.
- Performs nursing care and other field staff functions as necessary.
- Other duties as assigned.

This is full-time hourly in-office position, if you're an LVN you will have occasional field visits.
