**Position Summary**:
The Case Manager assists clients in breaking the cycle of recidivism by providing full wraparound case management and employment services with a heightened focus on self-sufficiency. The Case Manager facilitates all service and supportive service referrals based on the unique needs of each program participant through outreach with Florida Department of Corrections and community providers. The Case Manager works with a multi-tiered support system to help participants develop a comprehensive plan and work toward attaining their career pathway goals. The Case Manager follows up on all service referrals and collaboratively works with the participants to ensure they can successfully transition out of the prison system.

Salary Range: $38,500 - 43,000

**Essential Duties and Requirements**:

- Develops and disseminates information to groups and individuals to promote increased awareness of OIC of South Florida and its programs.
- Serves as a liaison and contact person with other agencies to ensure coordinated and comprehensive delivery of services to participants.
- Submits timely and accurate reports and data related to the program and participants.
- Develop and maintain working knowledge of community resources related to case management
- Coordinate wrap-around and support services for participants to overcome barriers to training completion and employment placement.
- Support the efforts of the internal and external stakeholder meetings, including participation in committees and identified working groups
- This position will require independent travel between agency resource centers and other venues as required by position. Must maintain valid DL and insurance coverage on personal vehicle at all times during employment.
- Conduct participant intakes, eligibility verification, and document according to program guidelines.
- Provide ongoing case management to assist participants in identifying and accessing resources that will help them mitigate barriers and navigate available career opportunities
- Implement trauma informed care and harm reduction best practices when needed.
- Maintain accurate and complete files for program participants as related to services and activities.
- Administer multiple client assessments per program guidelines and provide accurate and timely feedback.
- Utilize assessment results to assist in the development of the participants’ goal sheet.
- Conduct regular check-ins to ensure participant success.
- Participate in regular staff, case staffing, in-service, and other meetings
- Advise immediate supervisor and peers of needs, trends and issues within the area of responsibility and recommend activities to address them.
- Assist in conducting outreach and recruitment processes consistent with program goals and objectives.
