**Job Description**:
Key responsibilities include processing referrals and admissions, as well as communicating directly with clients and vendors to ensure customer satisfaction.

The Intake Coordinator is responsible for the following but not limited to:

- Registering, and admitting patients at the facility. Creating patient records.
- Accepting incoming referrals and entering them into tracking software.
- Managing the process of converting a referral into an active home care case.
- Performing insurance verification on all incoming referrals and obtain necessary authorizations.
- Ensuring referral forms are completed correctly, and assisting with securing any supporting documentation needed.
- Promptly advising appropriate staff of new admissions, and entering the cases into scheduling system.
- Maintaining accurate, current records of admissions, discharges and other changes to the caseload.
- Acting as liaison with family members and referral sources.
- Acting as a point of contact with contracted vendors.
- Maintain confidentiality and adhere to HIPAA regulations.
