**Social Worker
- Home Hospice**

**Full-Time**

**Day Shift (8am - 5pm)**

**Every 5th weekend**

**1-2 holidays/year**

This role works in collaboration with the Interdisciplinary Team to provide social work interventions, education, counseling and support services to address the identified psychosocial needs of patients and caregivers. **MSW required, LSW or LCSW is also required. It is the expectation that license is obtained within 2 years of hire. Two years’ experience in the health care setting preferred.**
