**Duties: What you will do**:

- Continue to develop Unity Hospice’s palliative care program
- Manage a caseload of palliative patients
- Assist with all aspects of patient care including: diagnosis, treatment, medication management, assessments, and consultations
- Maintain comprehensive knowledge in field of hospice and palliative care
- Travel to patients wherever they reside: Home, nursing facility etc.

**Requirements: What you need to have**:

- Valid Advanced Practice Registered Nurse (APRN) license
- Valid Registered Nurse (RN) license
- NPI number, or ability to obtain one
- Controlled Substance License and DEA Number (preferred)
- Willingness to enter into collaborative agreement with Hospice Medical Director
- Driver’s license, insurance and vehicle
- 1-2 years of nursing experience (preferred)
- Utmost compassion and integrity
