**Description**:
**JOB SUMMARY**
A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

**JOB DUTIES**
1. Actively demonstrates the organization’s mission and core values, and conducts oneself at all times in a manner consistent with these values.
2. Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information.
3. Improves access to health and human services for patients through in-person interactions, telephone contacts, and coordination of referrals.
4. Provides basic education regarding chronic health conditions in a way that promotes understanding and self-management by the client.
5. Facilitates or conducts enrollment in available federal, state and local programs to increase access to health care and support services. Examples include: health coverage via the marketplace, Medicaid, Medicare, social security benefits, food, and housing services.

6. Provides in-service education to service providers relative to healthcare customs/beliefs of the target population and works cooperatively with other community agencies to promote culturally appropriate services.
7. Maintains frequent contact with community agencies serving vulnerable populations and participates in community meetings.
8. Community Care Coordination
a. Provide effective care coordination for individuals
b. Build caring relationships with participants to promote positive health and social service outcomes
c. Adhere to an ethical code of principles to strengthen the professional role of the CHW
9. Individual & Community Advocacy - Serves as a community liaison between target populations and agencies and providers
10. Teaching Strategies for Change
a. Assess stage of change and provide education appropriate for readiness
b. Utilize a variety of teaching strategies, including verbal, handouts, and/or video based on participant learning style
11. Community Resources
a. Identify and refer people from targeted populations with basic needs
b. Utilize community resources and their referral processes
c. Identify and utilize entitlement participant program services
12. Organizational & Personal Skills
a. Identify the emotional dynamics involved in CHW work and utilize a personal and professional support system to cope with these dynamics

**JOB SPECIFICATIONS AND CORE COMPETENCIES**
Education: High school diploma or equivalent.
Licensure: Basic Life Support (BLS) certification is preferred.
Experience: One to three years of community involvement experience. Meets additional state/local requirements.
Other Job Requirements:
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively interview clients and accurately record medical information. Ability to effectively present information in one-on-one and small group situations to clients, and other employees of the organization. Ability to effectively relate pertinent information with medical and social service professionals.
Collecting & Reporting Confidential Information - Understands and is capable of using multiple technologies including but not limited to desktop/laptop computers, mobile devices and tablets in order to accurately and in real-time capture relevant social determinant information for the medical record
Must possess a comprehensive knowledge of the local community.
Work requires driver's license in good standing, the ability to drive a vehicle or have access to reliable transportation.
Work requires ability to communicate orally and to hear, or communicate with target population. Bi-lingual skills to work with patients in their native language are preferred.
Work requires proofreading and checking documents for accuracy.
