We are actively seeking a qualified and dedicated Pharmacist to join our remote team with a focus on Long-Term Care (LTC). The ideal candidate will possess expertise in long-term care pharmacy practice, excellent communication skills for remote collaboration, and a commitment to providing exceptional pharmaceutical care.

Preferred Qualifications:
  • PharmD degree from an accredited pharmacy program
  • Long-Term Care pharmacy experience
  • State pharmacy license in good standing (remote work eligibility)
  • Clinical pharmacy expertise with a focus on geriatrics
  • Strong analytical and problem-solving skills
  • Excellent communication and collaboration abilities for remote interactions
  • Familiarity with electronic health record (EHR) systems used in LTC settings
  • Dedication to ongoing professional development and continuous improvement
Responsibilities:
  • Provide remote clinical pharmacy services tailored to the unique needs of long-term care residents
  • Collaborate remotely with healthcare teams, including physicians and nursing staff
  • Conduct remote medication reviews and consultations for optimal therapeutic outcomes
  • Ensure compliance with regulatory standards and best practices in long-term care pharmacy
  • Utilize electronic health record systems for documentation and communication
