*JOB DESCRIPTION AND PERFORMANCE STANDARDS**Position Title: **Certified _Occupational Therapy Assistant_ * Responsible To: _Administrator/Therapy Coordinator_Purpose of this position The purpose of this position is to treat and document care for residents with impaired ability to perform at an independent level due to disease process under the supervision of an occupational therapist; participate in the interdisciplinary team meetings as required. *Qualifications** Education: Graduate of an accredited program of Occupational Therapy Assistant program, as a minimum, of an Associates Degree. Must have a valid Certified Occupational Therapy Assistant license or equivalent and an American Occupational Therapy Certified Board Certificate. * Knowledge: Must know basic personal care of residents in long term care setting. Must maintain environment in orderly manner and be able to communicate effectively with the residents, visitors, supervisors and co-workers. Must be able to maintain confidentiality of resident conditions and affairs. Must have general knowledge of local, state and federal rules and regulations. Must be able to read forms. Must be able to use office/medical equipment. Must be able to visualize the residents status during treatment. Must be able to communicate person to person and via telephone. Must be able to manipulate the equipment. Must be able to write. Must be able to follow verbal and written instructions. * Physical: Must be able to lift one (1) to twenty-five (25) pounds from floor to waist, up to ten (10) pounds from shoulder to overhead. Must have ability to carry up to twenty-five (25) pounds with assistance as needed. Must be able to occasionally twist, stand, walk, sit, climb, kneel or crawl as needed possible for lengths of time. Delegation of authority Authority is delegated to the individual in this position to: * follow physicians orders, facility policies and procedures to treat residents and complete required documentation for care provided * participate in discharge planning when appropriate *Duties and Responsibilities** Assist the Occupational Therapist in planning, development, organization, evaluation and directing of occupational therapy activities, in accordance with current rules, regulations and guidelines. * Provide appropriate treatment to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilities. * Provide appropriate treatment, preventative measures and adaptive equipment to prevent decline in residents function. * Provide appropriate resident or caregiver education to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilities. * Maintain timely and accurate resident reports as required by federal, state and local regulatory agencies and third-party payers. * Provide adaptive equipment and preventative measures to prevent decline in residents function. * Participate in discharge planning, development and implementation of resident care plans, resident assessments etc. as necessary. * Assist in instructing resident and family members in home programs as well as care and use of adaptive equipment. * Maintain treatment records, resident files and progress notes as required. * Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of the resident. * Participate in development and implementation of resident care plans, including quarterly and annual reviews. * Brief resident on procedures involved in occupational therapy and prepare necessary equipment. * Assist in transporting residents to and from the occupational therapy area. * Instruct and supervise resident during therapy. * Consistently and accurately perform thorough evaluations according to protocol. * Teach basic living skills to resident as necessary. * Abide by the residents participation in treatment decision, honor residents refusal of treatment request. Notify Therapy Coordinator of refusals. * Recommend modifications and changes in the residents therapy treatment as indicated. * Ensure that notes are informative and descriptive of care provided and residents response to care. * Ensure that occupational therapy activities and procedures are those indicated in the care plan. * Ensure that therapy area is maintained in a clean safe manner for resident comfort and convenience. * Wear personnel protective clothing when performing services that in involve potential exposure to blood or body fluids. * Utilize all department equipment and supplies in an efficient manner that promotes desired therapy outcomes. * Provide necessary material/equipment for resident to perform required therapy. * Accept assigned duties in a cooperative manner. * Follow all facility policies and procedures. * Assume Quality Assurance and Board Meeting duties as assigned in addition to submitting a report monthly. * Come to work in neat, clean attire and consistently present an appropriate professional appearance. * Come to work as scheduled and consistently demonstrate dependability and punctuality. * Consistently work cooperatively with residents, residents representatives, facility staff, physicians and ancillary service providers. * Attend and participate in initial orientation and regularly scheduled in-services. * Attend and participate in mandatory in-services as assigned. * Agree not to disclose residents protected health information and promptly report suspected or known violations of such disclosure to the Administrator and/or Compliance Officer. * Serve on various committees of the facility as appointed. * Develop and maintain a good working rapport with other departments within the facility. * Perform any function delegated by the Administrator or designee. *I have read and understand the functions and requirements of this position.
