We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our home health care team. The LMSW will provide comprehensive social work services to patients in their homes, addressing psychosocial needs, facilitating access to resources, and working collaboratively with the health care team to support patient care plans. This role is critical in ensuring the overall well-being and quality of life for patients and their families during the course of treatment and recovery at home.

*Key Responsibilities:*
* *Patient Assessment and Care Planning:*
* Conduct comprehensive psychosocial assessments of patients and their families to identify emotional, social, and environmental strengths and problems related to their illness and treatment.
* Develop individualized care plans that address identified needs, integrating patient and family input.
* Reassess and modify care plans as needed, in collaboration with the health care team.
* *Counseling and Support:*
* Provide counseling and emotional support to patients and families to help them cope with the impacts of illness, treatment, and potential changes in lifestyle.
* Facilitate family meetings to ensure open communication and effective problem-solving.
* Offer crisis intervention and supportive counseling in cases of acute distress or emergencies.
* *Resource Coordination:*
* Connect patients and families with community resources and services such as financial assistance, support groups, and transportation.
* Assist with accessing and navigating health care systems and insurance benefits.
* Advocate for patient rights and needs within the health care system and community.
* *Team Collaboration:*
* Work closely with nurses, physicians, therapists, and other members of the health care team to ensure coordinated care.
* Participate in case conferences and contribute to the development of interdisciplinary care plans.
* Provide social work consultation to other team members to enhance patient care.
* *Documentation and Compliance:*
* Maintain accurate and timely documentation of all patient interactions, assessments, care plans, and interventions in compliance with organizational policies and regulatory requirements.
