*Responsibilities:*
* Coordinating and managing home health care services for patients.
* Assessing patients' health needs and developing personalized care plans.
* Providing direct patient care and managing treatment plans.
* Educating patients and their families about home health care practices.
* Ensuring compliance with healthcare regulations and safety protocols.
* Maintaining accurate patient records and documentation.
* Mileage (per government rate) is in addition as travel is needed to see patients
* Collaborating with healthcare providers to ensure comprehensive patient care.
* Monitoring patients' progress and adjusting care plans as needed.
* Scheduling and conducting regular home visits to assess patient conditions.
* Managing patient discharge and follow-up care.

*Requirements:*
* 2 years of home health experience required.
* Registered Nurse (RN) with active licensure.
* BLS (AHA).
* Experience with Cerner or similar computer charting systems.
* Strong knowledge of home health care protocols and procedures.
* Ability to work independently and manage multiple patients.
* Excellent communication and organizational skills.
* Strong attention to detail and problem-solving abilities.

Pay: $58.00 - $60.00 per hour
