Our groundbreaking Hospice and Palliative Care programs offer a broad range of services in both in-patient facilities and the community to help patients and their families ease the burden of managing a life-limiting illness or end-of-life care.

Working on-site at a major medical center, the Hospice Care Planner is responsible for evaluating and establishing a plan of care for patients referred for Hospice services. The Hospice Care Planner will facilitate admission of referrals upon discharge from the hospital to a hospice program that meets the needs of the patient and family members. Must have extremely strong Clinical experience. Per Diem Rate is $55/hour Weekday, $60/hour Weekends.  Saturdays required.

  • Licensed with current Registration to practice as Registered Professional Nurse in NYS is required
  • Bachelor's Degree preferred
  • Must have a minimum of two years experience in home care nursing with proficiency in physical assessment and interviewing skills
  • Must be client-service focused with excellent written and verbal communication skills
  • Advanced critical thinking and problem solving skills, plus the ability to work independently is essential
