We are seeking a dedicated Home Health Registered Nurse or Licensed Practical Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

Qualifications:

Must possess current Illinois RN or LPN licensure
Home Health experience required
Current CPR
Valid IL driver’s license and auto insurance policy
WellSky (Kinnser) experience a plus
Must possess excellent clinical nursing and customer service skills
