*Job Description: *The LPN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching/visit duties to ensure the highest quality of care.

*Duties and Responsibilities:*
* Performs client visits as needed with authorization by the Division of Senior and Disability Services, as well as reassessment visits for In-Home Services and Advanced Personal Care (APC) clients.
* Weekly Medication set up
* Monthly Diabetic Nail care
* General Health Evaluation
* Skin Assessments
* Nursing Assessments

*Qualifications, Knowledge, Skills, and Abilities:*
* Graduate of approved School of Professional Nursing.
* Current state Registered Nurse or Licensed Practical Nurse license.
* Minimum one (1) year of experience in professional nursing within the last three (3) years. Prefer a minimum of six (6) months of experience with Medicaid In-Home Services nursing.
* Geriatric nursing experience helpful.
