ProHealth Partners has excellent opportunities in the Toledo, OH area. We are in need of a Full Time Physical Therapist to work a 3-day weekend schedule (Thursday, Friday, and Saturday) along with an additional 4th day during the week of your choosing. This opportunity is perfect for the candidate that has strong documentation abilities with an emphasis on OASIS knowledge. *The position rates are based on experience in the home health setting.
Our Care Coordination team provides our therapist with valuable resources such as scheduling and documentation support so that you are able to focus on what matters most, patient care.
$10,000.00 sign on Bonus

Job Responsibilities:
Supervision to Asst. therapy staff
Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll
Keeping open communication with patients and home health agencies to promote a positive and caring experience
Facilitating communication with Physical Therapist Assistant to manage patient care
Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork
Attend meetings and case conferences
Job Requirements:
Current Physical Therapist Licensure
Bachelor’s degree in Physical Therapy
Ability to travel to patient’s home or facility
Strong communication skills
Home healthcare experience preferred
This position is 10 hours 4 days per week
This position requires daily commute to patients’ homes and facilities in a 40–mile radius of Toledo, OH.
