Advanced Home Health and Hospice of Cincinnati is looking to add a PRN Speech Therapist to our Home Health team. This position will plan and provide Speech therapy services to clients in their homes in accordance with the Plan of Care and Agency policies and procedures. This position will demonstrate the ability to communicate well with other clinicians and partners to ensure quality patient care is always delivered.
Graduate of a Speech Therapy program.
* Possesses current registration or license as required by the state of practice.
* Minimum of one (1) year experience in a health care facility or equivalent experience preferred.
* Performs speech therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.
* Documents all findings, plans, interventions, and client progress (outcomes) on designated forms and submits per Agency guidelines within (48) hours.
* As well as all other Agency staff on changes to the care plan. Plans, teaches, supervises, and counsels clients and families about their speech therapy needs, in order to promote symptom management of disease process as well as overall improvement or recovery. Assist client and family with community resource needs.
* Uses OASIS assessment tools and outcome reports to improve therapy services and client outcomes.
* Demonstrates knowledge of safety/infection control practices by compliance with agency policies and procedures.
* Assesses safety of environment and takes initiative to help prevent accidents and promote safety.
