Job Description

Pride Health is hiring a LPN-HOME HEALTH to support our client’s medical facility in Massachusetts and other locations. This is a 13-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization

Job Summary:

As a Licensed Practical Nurse (LPN) in home health, you will be responsible for providing essential nursing care to patients in their homes. Your duties include monitoring vital signs, administering prescribed medications, wound care, and educating patients and their families on managing chronic conditions. You will collaborate with physicians and other healthcare professionals to develop and implement personalized care plans. Your role is vital in improving patient outcomes by ensuring the highest quality of care in a comfortable, familiar setting. Excellent communication and empathy are crucial for this position, as you will often work independently and directly with patients and families.


Key Responsibilities:

  • Administer medications and treatments as prescribed, ensuring patient safety and adherence.
  • Monitor and document vital signs, symptoms, and patient progress during home visits.
  • Provide wound care, including dressing changes and infection prevention.
  • Educate patients and families on managing chronic conditions and post-discharge care.
  • Collaborate with physicians, case managers, and other healthcare professionals to develop care plans.
  • Assist with mobility, personal hygiene, and activities of daily living (ADLs) for patients.


Qualifications:

  • LPN license
  • BLS from AHA
  • Strong critical thinking, communication, and stress management skills.

  • Location: Massachusetts and other locations
  • Job Type: 13 Weeks Contract
  • Shifts: Day shift of 8 hours
  • Weekly Pay: $ 1600 to $ 1800 for travellers and $1600 for locals

*Offered pay rate will be based on education, experience, and healthcare credentials.
