How you make a difference:

The Discharge Planner coordinates and implements discharge planning services including identifying local resources and coordinating appointments with programs and/or providers as needed. Discharge Planners may also provide individual and/or group psychoeducation for individuals prior to releasing into the community.

Key Responsibilities:
Provides discharge planning services starting at intake, including MAT, medical, and behavioral health programs, following contract, accreditation, and policy standards.
Conducts individual and group psychoeducational sessions for individuals scheduled for release.
Builds and maintains relationships with community agencies offering services for mental illness, substance use disorders, and serious medical conditions.
Facilitates applications for financial assistance and coordinates resumption of benefits and insurance, ensuring medication and medical needs are met upon release.
Maintains effective working relationships with facility staff and documents services in the health record, while adhering to security regulations and confidentiality requirements.

Qualifications & Requirements:

Education

Bachelor’s degree in social work, psychology, or related field from an accredited college or university or nursing license; LPN or RN, preferred.
High school diploma with care coordination and correctional experience will be considered.

Experience

Experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental health needs. Previous correctional experience preferred.

Licenses/Certifications

Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.
