The Work of the ON CALL Registered Nurse HOSPICE Case Manager Includes

Patient Care

Schedules, assigns and directs staff on appropriate patient visits.
Demonstrates competency in skills required by the agency for patient care.
Communicates significant findings, problems, changes in condition or environment to the CNO, clinical manager, physician, and other members of the care team.
Assesses and reassesses physical, environmental and emotional factors to determine patient needs.
Utilizes the patient's plan of care to maintain continuity of care by communication with appropriate staff.
Answers all incoming calls in a manner appropriate to the nature of the call, makes follow-up calls on patients as necessary. Handles referrals and inquiries for services, rapidly identifies the caller's needs, taking proper action to assure caller's needs are handled.
Maintains flow of communication between in-house staff, field staff and other departments, for coordination of activities and services.
Conducts follow-up with by phone or fax to ensure quality of care and notifies appropriate personnel.
Initiates appropriate nursing procedures.
Works in cooperation with the family/caregiver and care team to meet the emotional needs of said parties.
Communicates with physician regarding patient needs and reports changes in condition; obtains orders as needed.
Develops, prepares, and maintains patient care records with accuracy, timeliness and according to policies. Submits accurate documentation in a timely manner.
Utilizes a combination of agency resources and rationally recognizes standards of practice to achieve excellent patient care.
Participates in agency performance improvement initiatives.
Utilizes teaching materials in staff and patient/family education.
Minimizes non-productive time and fills slow periods with activities that assist in meeting the current and future needs of the agency.
Gain knowledge of and availability to perform patient intakes and information visits as needed including explanation of benefits/Medicare, complete physical assessment, completion of all pertinent paperwork, and communication of new patient status.
Reports concerns to appropriate personnel.
Facilitates development of a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.

Communication

Completes, maintains and submits accurate and relevant notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
Educates the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
Facilitates Provision and maintenance of a safe environment for the patient.
Facilitates the patient and family/caregiver and other team members in providing continuity of care.

Additional Duties

Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
Actively participates in quality assessment performance improvement teams and activities.
Other duties as assigned.

POSITION QUALIFICATIONS

Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
Current licensure in Missouri and Kansas, as well as, American Heart Association CPR Certification.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Bachelor's degree, with one (1) year of home health care and/or hospice care experience preferred.
Management experience not required. Responsible for supervising nursing
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
This position requires the ability to perform, with or without reasonable accommodation, the following essential functions:

Ability to sit, stand, bend, lift, pull, turn, and stoop.
Ability to provide health care to patients according to patient care plan.
Ability to reposition, lift and turn patients.
Ability to access, input, and retrieve information from the computer.
Ability to communicate effectively with staff, patients and patient family members, ans management.
