Job Description:

The LPN / LVN participates, coordinates and provides direct nursing care in a manner which ensures participant's satisfaction and retention. Assists with medication orders and delivery, coordinating with the physician, RN, transportation and home care. Assists physicians and nursing staff with a variety of tasks, including but not limited to vital signs, wound care, medication administration/management, treatments, patient teaching, venipuncture and microcoagulation, catheter care foot care, administration/reading PPD tests and participant set up for physician visit.

Required Qualifications:

  • Education sufficient for licensure
  • within 30 days of hire: National Provider BLS - American Heart Association.
  • upon hire: Washington Licensed Practical Nurse.
  • upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. (Home Health positions).
  • 1 year Long term care or related geriatric experience.
  • Experience working with the frail and elderly

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
