Social Work Case Manager

The base pay for this position is $20 to $24 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. *Responsibilities:*
* Manage a caseload of clients.
* Connect clients with needed medical, mental health, and/or substance use services
* Collaborate with other professionals to identify wrap around services for clients in outpatient behavioral health and other social service programs
* Connect clients with services including insurance, housing assistance, educational and vocational training, supported employment, specialty medical care and other social service needs.
* Accompany clients to appointments and perform home visits as needed
* Contribute to case conferences
* Organize group trainings to support clients as they work to integrate into their communities and lead productive, satisfying lives.
*Qualifications:*
* Bachelor's degree in social work, human services, healthcare, or a related discipline
* Minimum of two years experience in the human service field
