The Transition of Care LPN provides a variety of functions in the care management department pertaining to patient care, healthcare needs, identifies appropriate resources and cost-effective solutions.

Telecommuter position, but must be able to independently travel to Farmington, CT for as needed training/meetings
* Schedule: Full time, 40 hours/weekly, Monday through Friday, 8:30AM - 5:00PM

If you are located in Connecticut, you will have the flexibility to work remotely* as you take on some tough challenges.

Member Care Coordination: Works closely with physicians involved with members to coordinate care plan communication; collaborates with multidisciplinary team to maintain and implement up-to-date coordinated care; communicates with all members of the healthcare team on behalf of the member.
* Clinical Improvement: Proactively participates in the development and deployment of Coordination of Care activities with the goal of improving the clinical experience for referred Members and the referring physician.
* Communicates with members of the Care Team as appropriate to coordinate the identified Member and physician's needs.
* Educate Member and care team participants on community/health plan benefit services available.

Graduate of accredited school of nursing
* 2+ years of nursing case management experience
* Proficiency with navigating multiple computer programs (including but not limited to Microsoft Products, Adobe)
* Resident of the state of Connecticut
* Ability to travel independently to Farmington CT for training and meetings as needed. Possible travel to other locations at times.

Experience with Electronic Health Records (Epic)
* Experience in a remote work setting

Connecticut Residents Only: The hourly range for this role is $21.Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
