Job Description
Job Title: Skilled Nursing LPN Charge Nurse

Job Specialty: Nursing

Job Duration: 13 weeks

Shift: Variable shifts, 12 hours (6 a.m. - 6:30 p.m. or 6 p.m. - 6:30 a.m.), rotating weekends and holidays

Guaranteed Hours: 36 hours per week

Experience: Prior work experience in a similar or related position

License: Licensure for the state of practice is required

Certifications: BLS from American Heart Association

Must-Have: Graduate of an accredited school of nursing

Job Description:

- Assist in formulating a plan of care for residents according to location standards, procedures, and individualized resident care plans.
- Provide nursing care to residents in accordance with physician orders and regulations.
- Update the plan of care as needed.
- Observe and document resident conditions and care provided accurately in the EMR.
- Ensure physicians are notified and orders are processed promptly.
- Share information and report on residents' conditions.
- Respond promptly to resident call lights.
- Direct and assist with resident and family/significant other education.
- Notify families of condition changes and other significant information.
- Handle linens properly and dispose of soiled linens, clothing, or supplies following regulations and guidelines.
- Use standard precautions, employ personal protective equipment, and observe transmission guidelines, including washing hands between resident cares.
- Report and assure follow-up of signs and symptoms of resident infections.
- Comply with all related laws, regulations, company policies, and procedures pertaining to the position.
- Fulfill obligations under the organization's corporate compliance program.
- Adapt to changes in duties, responsibilities, and activities as needed.

Maintain effective communication and ensure adherence to safety guidelines within a moderate noise level work environment. The role requires lifting up to 35 lbs. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
