Take Your Therapist Home

As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program

In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where you as the clinician work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes

You will be primarily in the home health setting with dedicated time and caseload support for the SNF. p> Rivetus has been serving the geriatric Post-Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians. Our perspective is unique in providing continuity of care across the health care continuum in the Skilled Nursing, Home Health, and Outpatient settings.

Facility Location:
Belle Fountain - This is a hybrid role providing Facility based therapy services and local Home Care based therapy services. Facility services are paid at an hourly rate, Home care services are paid per visit.

Schedule:
Monday - Friday, Full Time

General Summary:
In this role you will provide physical therapy services to patients impaired by physical dysfunction, including assessment, treatment planning and implementation of treatment plans. Provides supervision for PTAs and aides as assigned. Completes appropriate documentation and adheres to policies, procedures and current standards of clinical practice.
Conduct physical therapy screenings and evaluations to determine patient physical status.
Provide physical therapy treatment consistent with the professional and ethical standards of the APTA for Physical Therapists.
Provides direct patient care in accordance with productivity standards by use of graded activities to facilitate the restoration of physical and functional abilities.
Provides documentation to maintain quality of the medical record as it relates to accuracy, continuity of care and reimbursement guidelines.
Participates in CQI activities as required ensuring continuous quality of care.
Participates in company meetings and additional activities directed to facilitate effective departmental operations.
Required Qualifications:
Successful completion of an accredited Physical Therapy program.
Ability to react effectively to the needs of patients and their families.
