Seva Hospice is a locally owned and operated company of experienced hospice providers with a desire to serve the communities we live and work in. We are looking for an Admission Registered Nurse to join our dynamic team, serving Stanislaus county to provide care and support to terminally ill patients. This position is full time, Tuesday - Sunday, 11:00am - 7:00pm with weekend on call rotation.

In this position, you will guide patients and families through the hospice assessment and admissions processes and play a pivotal role in the educational process for patients, families and caregiver. Your presence and services should bring comfort to the person who faces the end of their lives and help them reach acceptance and peace of mind. For this role, you need to be a registered nurse who sees the value of hospice care in alleviating pain and suffering. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. We expect you to be knowledgeable in caring for the terminally ill, providing expert symptom management and providing emotional support. The goal is to help patients live their final days with dignity and comfort.

Skills

Minimum one year nursing experience, hospice or home health preferred
Knowledge of nursing care processes and medication guidelines/side-effects
Committed to safety precautions and high ethical standards
Proficient in reading/writing English, bilingual a plus (Spanish preferred)
Sound judgement and attention to detail
Excellent communication skills and a compassionate nature
Cool-tempered and patient
Emotional/mental stability and stamina

Additional Requirements

CA Registered Nurse license required
1 year nursing experience required
Hospice experience preferred
Current CPR certification required
Must have reliable transportation and auto insurance

Responsibilities

Make initial visits to patients to complete comprehensive assessment and determine hospice needs
Collaborate with physicians and the Interdisciplinary Team to formulate and review care plans
Make frequent visits to the patient to evaluate their condition
Provide support and comfort according to individual patient’s needs
Work with caregivers to plan and execute excellent patient care
Help in administering medication
Alleviate symptoms to the patient's desired level of comfort
Notify a doctor for a patient’s condition when deemed necessary
Assist patients and their families with paperwork
Maintain accurate records
Help relatives cope with the upcoming loss and provide post-mortem support
Participate in after hours on-call, holiday and weekend rotations
