Responsibilities:
  • Provide 5-Star care to our amazing residents
  • Follow established Ohio Board of Nursing and FVD standards and procedures to ensure quality services are delivered in a timely, friendly, professional manner at all times to clients and coworkers. Performs nursing duties as required, ordered by physicians or requested by residents, which may include, administering and setting up medications, changing dressings, glucometer readings, taking vital signs, administering CPR and emergency care.
  • Respond to emergency calls from residents and assesses the situation; delivers appropriate care and/or call for assistance; keeps appropriate documentation.
  • Perform a resident physical assessment in time of acute illness. Reports subjective and objective data to primary care provider or calls 911 in emergency.
  • Assess and follow-up as indicated on each resident to ensure proper care; supervise aides to ensure policies, procedures and care plan are followed, when assigned.
  • Provide medication management to residents.
  • Perform nursing duties: wound care, infections, urostomy care, catheter insertion and care, and other nursing duties prescribed.
  • Coordinate care with resident, physician, and family members. Take off orders, make referrals and order lab work.
  • Be a resident advocate and meet with resident and/or family members to provide information on services and provide resources. Discuss care and service options and interventions available in the IL setting as well as the continuum of care. Understand the resident should be included in care decisions. The Resident Care Manager is responsible to identify and articulate the individual's wishes, values and preferences and incorporate into the care plan if possible. Discuss benefit versus risk for aging in place services.
  • Explain all procedures, treatments, medications and billing to client and family, as appropriate.
  • Conduct assessment of resident needs and develops care plan for companion and/or AIL services in a timely manner. Follow up with Companion, AIL and resident to ensure proper care is delivered and that the care plan is being followed.
  • Assess and review resident status as requested by resident, family members, doctor, or FVD staff; contact appropriate persons (e.g., doctor, family member, emergency service); make necessary changes to care plan.
  • Admit residents into Home Care Services program. Complete required admission forms and follow process by entering admission data into Point of Care clinical electronic record.
  • Demonstrate consistent and clear communication with staff, family, and primary care provider in regard to resident condition, changes, and challenges.
  • Track resident supervisory visits for compliance and perform supervisory visits in resident home to assess resident satisfaction of service and modify plan of care/service as needed.
  • Demonstrate proactive decision making in regard to resident needs and services needed to ensure resident safety, dignity, quality of care and independence.
  • Reports and develops a plan when resident condition deviates from baseline.
  • Collaborate with Registered Nurse to evaluate resident care plan and resident response to care and progress towards goals. Revise care plan as necessary.
  • Ensures all resident needs are assigned daily and daily resident schedules are accurate and up to date. Communicate staffing needs to Office Coordinator and follow-up to ensure resident needs are met. May need to work week-end shift if there is not nurse coverage.
  • Administer employee Mantoux and Hepatitis B injections and record keeping.
  • Maintain current CPR certification (as required) and professional licenses. Obtain required CEUS for licensing requirements, to remain current in field and for professional growth, with emphasis on care for the elderly.
  • Promote teamwork within department and between departments to ensure smooth operations and quality service to residents and coworkers, including assisting when and wherever needed, treating all residents, coworkers and visitors as your valued customer, communicating with coworkers to keep them informed of events, changes and operations, in a friendly, professional manner while maintaining appropriate confidentiality.
  • Remain current on issues related to safety and health, especially with respect to the duties of LPN, which includes but is not limited to, knowing the SDS information on all chemicals used, knowing where SDS and other safety information and protective equipment is kept and how to use them, Infection Control procedures, and the policies and procedures of the Disaster Plan Manual.
  • Attend all mandatory in-services and meetings; participate in educational sessions, meetings and seminars as directed by the Home Care Services Manager.
  • Report all injuries, accidents and near misses to supervisor within 24 hours; assists in investigations to determine why the incident occurred and what can be done to prevent it from happening again.
  • Know, uphold and follow Resident Rights and Code of Conduct policies. Report to Home Care Services Manager or Clinical Manager any situation where the rights of a resident may have the probability of being violated. Only provide confidential information regarding residents to the Manager or RN on duty, or other health care professional as directed by your supervisor, according to an emergency situation.
  • Understand special needs of 65 and over group (e.g., possible loss of vision, hearing, memory, etc.); ability to effectively communicate and provide services for these age groups.

Qualifications:

  • Currently licensed to practice nursing in Ohio (LPN).
  • Valid CPR
  • At least six (6) months experience in medical or surgical nursing, preferably with geriatric patients; or an equivalent combination of education and experience.
  • General knowledge of the psychosocial and physical problems of the geriatric population required.
  • Ability to teach, supervise and motivate nursing staff to ensure appropriate care and rehabilitation.
  • Must have the ability to follow written and/or oral instructions and understand concepts of measurements.
  • Ability to communicate effectively in English, orally and in writing.
