Nurse Navigator, RN

Job Summary:
The Nurse Navigator serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Henry Mayo Newhall Hospital and in the community. Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians.
Licensure and Certification:
* Current CA RN license
* Current BLS provider card
* Patient Navigator Certification(s) desired or will obtain one of the following within 24 months of hire date:
1) BPNC Breast Patient Navigator Certification from NCBC National Consortium Breast Centers
2) CBCN Certified Breast Care Nurse from ONCC Oncology Nursing Certification Corporation
Education:
* BSN required
* MSN preferred
Experience:
* Five (5) years current acute care nursing experience, with recent oncology experience preferred.
* BSN/Bachelor's degree in related field may be substituted for 2 years' experience.
* MSN/Master's in related field may be substituted for 3 years' experience.
Experience:
* Strong leadership and critical reasoning skills in clinical decision making planning and organizing, initiative and ingenuity, creativity, independent decision making, problem solving, and ability to interpret data.
* At least five years of professional nursing experience in acute care or oncology with demonstration of competent clinical judgment and leadership ability preferred.
* Strong data management, experience in performance improvement measures preferred.
* Ability to work effectively with physicians and a interdisciplinary team toward achieving a common goal
* Effective two-way communication skills (listening and speaking); strong interpersonal skills with staff, physicians, patients and their significant others.
* Strong verbal and written communication skills.
* Strong computer skills, including use of Word, Excel and PowerPoint.
Physical Demands - Patient Care:
* Continuous standing/walking and occasional/intermittent sitting.
* Continuous use of bilateral upper extremities in fine motor activities requiring fingering, grasping, and forward reaching between waist and shoulder level to handle/operate medical equipment/devices.
* Frequent reaching above shoulder level and overhead.
* Frequent forward bending, twisting, squatting and kneeling; occasional climbing.
* Occasional repositioning and transferring patients weighing up to 200 pounds between bed, chair, and gurney.
* Occasional lifting and carrying equipment weighing up to 25 pounds.
* Occasional/intermittent pushing of gurneys, wheelchairs, bed and other medical equipment over tiled and carpeted surfaces.
* Continuous use of near vision to read medical equipment such as monitoring devices and reading documents and computer screens; hearing and verbal communication to interact with patients, co-workers, and other customers.
