* Part-Time .Days and Evenings
* On-site at Grand Itasca Clinic & Hospital in Grand Rapids
* The Visit Facilitator works primarily in family practice primary care and is responsible for supporting in person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. The visit facilitator gathers pertinent medical information from the patient and documents the information into the patient's medical record. This role also helps to ensure the exam rooms are stocked, equipment and supplies are available and assists with tasks that support the effectiveness and efficiency of the team.

Prepare patients for their appointments, including communicating with patients prior to their appointment to ensure medical records are collected and ready for the visit.

* Must be 18 years of age or older.


* Graduate of a Nursing Aide program, Emergency Medical Technical (EMT) program, and/or Health Unit Coordinator program. Certification and current listing on the Minnesota Nursing Assistant Registry preferred.
* Previous health care customer service experience preferred.
* Proficient with technology (computer hardware and programs, and smart devices such as phones and tablets).
* Demonstrated ability to adhere to and follow Grand Itasca Clinic & Hospital Mission, Values, Vision and policies.
