  1. Assess patients' needs using Motivational Interviewing skills.
  2. Based on assessment, Prepare, formulate, and evaluate an appropriate service plan for each patient.
  3. Directly assist patients attempting to access public and private benefits and entitlements.
  4. Collaborate with providers, nurses, behavioral health, case managers, and other colleagues to address patient needs and facilitate linkage to other Sixteenth Street Community Health Centers services.
  5. Review service plans at regularly scheduled intervals and provide phone and face-to-face interventions. Intervention is defined as a consumer-to-case manager's actual contact either by phone, in person, virtual, or other off-site locations.
  6. Prepare accurate records of each consumer, including evaluations, service plans, reviews, discharge reports, correspondence, and progress notes in electronic medical records.
  7. Develop and cultivate relationships with resources in the community to facilitate a smooth referral process and care coordination.
  8. Participate as an active member of the Social Services Department including staff meetings, in-service training, and workshops, department problem-solving exercises, and providing support to coworkers.
  9. Comply with A.I.D.E.T customer service culture.
QUALIFICATIONS:
  1. Bilingual (Spanish/English) required.
  2. Bachelor's degree in social work or human services-related field preferred. Consideration will be made for a suitable combination of previous academic and professional experience.
  3. Effective oral and written communication skills including the ability to network effectively within the healthcare and social services community.
  4. Motivational interviewing training and experience preferred.
  5. Ability to relate to and communicate well with people of various ages and from diverse ethnic, socioeconomic and cultural backgrounds.
