**Job Duration:** 13 Weeks
**Shift:** Monday to Friday, 8-hour shifts, Day shift from 8:00 am to 4:30 pm
**Guaranteed Hours:** Not specified
**Experience:** One year as a clinical nurse in a hospital required; nine months in home care preferred
**License:** RN License (NY License ONLY)
**Certifications:** BLS (any provider accepted)
**Must-Have:**
- Certified Home Health Care Experience
- Experience with OASIS
- EPIC or Netsmart knowledge preferred
**Job Description:**
- Develop and revise treatment and discharge plans in collaboration with patients, families, physicians, care teams, and third-party payors.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change assessments as per agency-defined timeframes.
- Process orders and inform physicians of patient needs and condition changes. Complete certification/recertification orders and discharge summaries.
- Promote an environment respecting patient privacy, confidentiality, and property.
- Initiate preventive and rehabilitative strategies.
- Provide services requiring substantial specialized nursing skills.
- Educate patients, families, and team members about disease processes, self-care techniques, and preventive strategies.
- Supervise LPNs to ensure compliance with care plans and promote patient safety.
- Direct, coordinate, evaluate, and supervise the quality of patient care services provided.
- Engage positively with patients, physicians, referral sources, and others.
- Support care management philosophy aimed at meeting individual health needs and promoting quality, cost-effective outcomes.
- Complete clinical records for compliance with legislation and service delivery within timeframes.
- Maintain current knowledge of all available community-based services.
- Show flexibility in visiting patients based on need, not geography.
- Ensure problem resolution at contact points.
- Obtain authorization and release information forms signed by the patient or responsible party.
- Attend patient care conferences for assigned patients.
- Pursue efforts to reduce or eliminate avoidable costs and errors.
- Respond to findings of quality management reviews/audits.
- Participate in peer review and quality assessment and performance improvement initiatives.
This position requires strong critical thinking, interviewing, clinical assessment, communication skills, initiative, independent judgment, and the ability to build and maintain professional relationships. Additionally, a current driver's license for the state is necessary.
